    Yes, the adult options are missing crucial information sections such as cultural/identity information. The child options don't make sense to send parents because they have extensive sections for the provider to fill out.

  2. I would like to be able to modify or upload my own custom forms i.e.- ROIs, Intake forms, Continuity of Care, Privacy Notices, etc.

    This would be great for teens and parents as well as for situations with divorced parents living in different households. We get this question all the time.
