Custom Intake Forms

Would like to be able to customize the Intake forms that are imported into the Jituzu Client Portal from My Clients Plus, or use my own Intake documents.

    Yes, the adult options are missing crucial information sections such as cultural/identity information. The child options don't make sense to send parents because they have extensive sections for the provider to fill out.

    I would also second that I'd love to be able to upload our progress note format and intake note format.
