I would like to be able to create my own fillable intake form to use with functional nutrition clients. I am a registered dietitian, however, the current templates do not match my needs or style of practice.

I love the variety of options of templates for notes and assessments. However, each clinician has their speciality and ways of assessing clients. I have created my own version of an intake assessment, through Word, that was similar to other assessment options provided, but not quite the same. It would be wonderful to be able to create and/or edit the assessments/note templates provided. At the end of the day, it is my license on the line for quality of the intake assessment and to be able to edit the options would be beneficial.
